1. What if I cannot submit my health information by the July 1st / January 13th deadline?

我们要求你尽一切可能的努力upload information and complete your health history questionnaire on Online Student Health by July 1st / January 13th. If you cannot adhere to the deadline due to extreme circumstances, please submit the information as soon as possible.

If you cannot submit a complete form by July 1st / January 13th due to a vaccine series not being complete or other circumstance, please submit as much health information as possible by July 1st / January 13th. Health information received after the deadline will be subject to a delay in review as information is reviewed in the order it is received.

If your uploaded information and health history have not been received by August 8th, the privilege of academic registration will be jeopardized. Students with incomplete health information will be required to present to Health Service during Orientation. This mandatory visit could jeopardize participation in Orientation Programming. Student OneCards (I.D. cards) will be deactivated, and failure to comply may impact your registration as a Wellesley student.

2. How will I know if you have received my health information?

Students can see what messages have been sent through the Patient Gateway. Look under Message Center---> Sent Messages. Forms are received immediately upon submission. It may take 1-2 weeks for us to review the forms.

4. What do I do if I cannot locate my immunization record?

  • You may be re-immunized. This will involve multiple vaccines given in multiple series.
  • You may have blood testing done to determine immunity. Health Services requires copies of all laboratory tests done.

7. How is the entering student checklist updated with my health information? How long will it take?

Your MyWellesley Checklist will reflect the status of your health information, indicating a red X when information is not complete and a green check when your health requirement has been completed. Information from Health Service is uploaded to the checklist weekly.

Please allow 1-2 weeks after form submission for the green check mark to appear (if your vaccines are complete). We will review documents as they arrive. There is no need to contact us regarding the status of your form prior to the 2 week mark.

Please make sure to check your email/voicemail regularly and respond to any messages regarding incomplete vaccinations as soon as possible. Failure to do this could delay your medical clearance.
